Cheiloplasty

Contours of lips play a decisive role in formation of an attractive image. According to modern standards, ideal lips look as follows: harmonious, young, slightly pudgy, clearly outlined, skin band of upper lip not prolonged, when smiling, upper teeth are bared by 2/3, lower teeth just slightly. One of the methods helping to achieve an ideal shape of lips is a plastic surgery of cheiloplasty..

What is cheiloplasty


Cheiloplasty is a method of plastic surgery that allows changing of shape of lips, their size and contours. . This is a real opportunity of correction of congenital or acquired defects and faults of lips that deform the appearance of a person and make him or her feel the inferiority complex.

Types of Cheiloplasty

Cheiloplasty can be aesthetic and reconstructive. Reconstructive methodology is recommended in cases of congenital imperfections, lip injuries, excessively enlarged lower lip, presence of various formations such as cysts, papillomas.

Among the indications for aesthetic cheiloplasty are asymmetric proportions of the mouth, age-related changes, which include reduction of lip volume, elongation of the upper lip, presence of wrinkles on it, as well as individual desire of patients to change the lip contour or increase their volume.

Surgical methods of cheiloplasty


It is necessary to mention "Bullhorn," V-Y technique, "Paris," Corner lift, DAO resection and reduction cheiloplasty among surgical techniques.

  • Average duration of surgery: about 20-30 minutes depending on the chosen surgical technique
  • Anesthesia: local or general (gas or intravenous anesthesia)
  • Immediately after the surgical intervention, the patient may leave home if feeling well

1. Bullhorn Technique

Directed towards correcting the upper lip. In English language this name sounds like “horns of a bull” as only a small area of skin is dissected resembling horns of a bull by its shape.

This method provides for a stable aesthetic effect as it makes it possible to reduce the distance from red stripe of upper lip to external nasal septum. After removal of skin flap, soft tissues are fixed in the direction of periosteum and edges of incision made are stitched to each other.

Due to reduction in volume of skin above the upper lip, the red stripe is lifted upwards and is slightly everted. This provides for a visual effect of a larger volume of the mouth as it becomes wider and lips become pudgy and tempting.

The incision can be made in two places depending on individual specificities of appearance of the patient. The first technique presumes an incision on the edge of red stripe of the upper lip. However, the final scar can be noticeable in this variant. Therefore, in most cases, surgeons prefer to apply the second technique – making an incision straight under the nose. In this case, the scar remains practically unnoticeable.

2. V-Y Technique

Presumes making several V-shaped incisions on mucosa of internal surface of lips. All incisions are then stitched together in the shape of letter “Y”. Their number and length determine the effect – lip is increased only in the middle or on the entire length.

3. Paris Technique

Similar to above described one. The only difference is making both vertical and horizontal incisions. Middle part of the lip is corrected with the help of this method as it acquires the shape of a heart.

4. "Corner Lift"

The "Corner lift" technique raises the drooping corners of the mouth. The positive effect is achieved by removing fragments of skin from the area above the corners of the lips and from the inside of the mouth. As a result, the lip line is leveled, and the outer corners of the mouth are lifted. The face looks younger, more cheerful, and attractive.

Over the years, it is characteristic for the corners of our lips to droop. This feature is also evident at a fairly young age due to the anatomical features of human structure.

5. "DAO Resection"

This method is performed to lift the corners of the mouth and correct marionette wrinkles (two long, deep folds from the corners of the mouth to the chin). Its essence lies in trimming the muscles (from the inside of the cheek) that control the movement of the corners of the mouth. Their drooping is associated with congenital features of muscle structure or age-related changes in the muscle that depresses the corner of the mouth (musculus Depressor Anguli Oris (DAO). In youth, this muscle is flexible, elastic, and long. Over time, due to constant participation in facial expressions, DAO becomes denser, thicker, less stretchable, and longer.

6. Reduction Cheiloplasty

Performed to reduce the lips by removing part of the skin cover. The incision is made along the Vermilion line - the line dividing the outer part of the lip and the inner, mucous membrane. After the surgical site heals, there are no traces of the operation, and the lips become thinner.

Indications for using this method of surgical intervention include: excessively voluminous lips; consequences of injuries; unsuccessful lip augmentation surgery in the past; lip asymmetry; patient dissatisfaction with the appearance of the lips and face in general.

Cosmetic change of proportions of the mouth can be achieved by means of injections. First of all, this is contour plastic of lips with the help of all kinds of dissipating bio gels on the basis of hyaluronic acid that provide for a temporary effect lasting for about one year on average.

The second variant of no surgery cheiloplasty is referred to as lipofilling. Here, own fat cells of the patient are injected into the lips of a patient as they are taken from other parts of the body. This procedure provides for a more stable effect. From my own experience, I can tell that correction of lips with hyaluronic acid gives a better result compared to lipofilling.

Contraindications to performing cheiloplasty include:

  • Shortened upper lip;
  • Presence of oncological diseases;
  • Poor blood clotting;
  • Tendency to form scar tissue;
  • Herpes exacerbation;
  • Nose shape where its tip is significantly upturned;
  • Pregnancy;
  • Chronic diseases in the exacerbation stage;
  • Diabetes.

Rehabilitation Period


  • Several hours after lip surgery, the patient can go home

  • Swelling, hematomas, bruises. During the recovery period, minor swelling, hematomas, and some discomfort are observed. All these symptoms usually disappear within 2 weeks

  • Pain sensations. As the anesthesia wears off, moderate pain and discomfort in the correction area appear, gradually subsiding. If there is significant pain, the doctor prescribes pain relievers

  • Lip sensitivity. Lip sensitivity may be disrupted due to nerve damage, which is considered normal. Sensitivity usually returns within a few weeks

  • Sutures. As a rule, stitches are removed on the seventh day. Over the next couple of months, a very small scar may be noticeable, which then smoothes out

After lip correction, it is not recommended to:

  1. Massage, rub, traumatize, or mechanically affect the surgical area

  2. Sleep on the stomach with the face on the pillow, as uneven pressure on the lips can lead to asymmetry for 5-7 days after surgery

  3. Use active mimicry and frequent lip movements (smiling widely, opening the mouth widely, kissing passionately) for 7 days after surgery

  4. Consume excessively cold or hot food, hard food (which can injure the lips, including seeds, nuts), and too spicy food (due to possible tissue swelling caused by increased blood flow to the manipulation site), drink cold and/or hot drinks for 1 week after lip surgery

  5. Consume nicotine, alcohol for 2 weeks after lip plastic surgery

  6. Visit a solarium, be exposed to direct sunlight, as ultraviolet radiation adversely affects the tissue healing process for 3-4 weeks after cheiloplasty

  7. Visit a sauna, bath, take a hot bath, and other procedures that provide thermal exposure to the body for 2 weeks after lip correction

  8. Engage in intensive sports for 4 weeks after the operation, which promotes hyperemia and increases tissue swelling due to increased sweating

Result Evaluation


The visible result can be seen immediately after the surgical procedure. However, it is recommended to evaluate the effect of lip surgery no earlier than six months. The final shape of the lips is formed depending on the individual characteristics of the body and the chosen method of lip plastic surgery.
